Your First Visit at Perry Smiles
You’ll receive a thorough dental exam and X-rays during your first visit. Our team will review your medical and dental history, discuss any immediate concerns, and create a treatment plan tailored to your needs. Our comfortable, family-friendly office is newly renovated, providing you with a welcoming space and high-quality care. We look forward to building a long-lasting relationship, addressing your questions, and offering you any dental patient resources needed for a smooth visit.
When you arrive, please bring the following information for your first office visit:
- X-rays from your previous dentist, if available (or ask them to forward these to us)
- A valid form of ID
- A list of any medications you take
- Information on any health concerns or past surgeries
- Your dental insurance card (if applicable)
